Comprehensive & Intensive Case Management and Housing Navigation
Aligning with the Partnership In care, Coordinated Entry System (CES) And the HUD
BESS or Homeless Program for State Homeless Outreach Program Principles that Outreach will be as follows:

"Delivered comprehensive case management, outreach services to unsheltered homeless youth, individuals and families, linking them to emergency shelter, permanent housing, to the chronically vulnerable homeless clients, runaway teens, individuals, veterans and families.

Assist individuals and families access permanent housing as rapidly as possible by assisting with documentations gathering (recover vital Records) State ID, Social Security Card, Birth Certificate, and assessments in connection to the requirements for housing stability.

Provide urgent, non-facility-based care and encourage clients to seek professional medical and mental health options

Collaborate closely with partners agency the Continuum of care (PIC) the Homeless Information Management System (HMIS), and create profiles in the Oahu Clarity system to protects, tracks, and monitors homeless clients information.

Assisting individuals, families, veterans and couples for application process obtaining an increase income through general financial assistance and Supplemental Nutrition Assistance and seek options and educate clients about the community resources .

Offer life management skills, telehealth, listening with empathy to their issues and concerns, monitored and track clients case profile and charts.
Therapeutic Allieance:
The foundation of Mana Pono Holomua is through faith-based therapy, a client-centered approach, motivational interview integrated with solution-focused brief therapy, and other supportive services. Mana Pono Holomua does not discriminate against other religious beliefs, but welcome it.
Housing First:
1) We will implement the aspect of housing first approach to clients who enter in our program and provide service that aims housing first (Clients does not have to have income to be house).
2) We will service clients who have legal issues involvement.
3) We will assist clients who are at risk of being homeless (Housing Diversion) divert homelessness
4) We will service clients "veteran" who are getting evicted to divert them from homelessness.
5) We connect our clients to vast community resources and referral to other non-profit organization
